How Nutrition Affects Fertility, Pregnancy and Childbirth

Of all the places nutrition quietly shapes a life, none matters more than the months before and during a pregnancy. The evidence here is unusually strong โ some of the clearest cause-and-effect relationships in all of nutrition science โ and yet the practical guidance rarely reaches the kitchen table. Here is what the research shows, and where better food planning genuinely helps.
Before conception: nutrition and fertility
Fertility responds to nutritional status in both directions. Significant underweight can suppress ovulation outright, and obesity is associated with reduced fertility and longer time to conception โ in all partners, not only the one carrying the pregnancy. Iron status, folate and overall diet quality before conception are all associated with reproductive outcomes, which is why clinical guidance treats the preconception months as part of the pregnancy itself.
Folate: the clearest case in nutrition science
Neural tube defects โ serious malformations of the brain and spine โ form in the first weeks of pregnancy, often before anyone knows a pregnancy has begun. Getting 400 micrograms of folic acid daily, starting before conception, dramatically reduces that risk; this finding is so robust that the U.S. mandated folic-acid fortification of grain products, and the Centers for Disease Control and Prevention recommends the daily amount for anyone who could become pregnant. It is the single best example of food preventing a birth complication.
During pregnancy: the load-bearing nutrients
Pregnancy roughly doubles iron needs โ iron-deficiency anemia in pregnancy is common and is associated with preterm birth and low birth weight. Calcium and vitamin D support fetal bone development; iodine supports brain development; adequate protein and appropriate weight gain support growth throughout. On the other side of the ledger, poorly controlled blood sugar raises the risk of gestational diabetes, which complicates delivery and affects both parent and child. These needs shift by trimester โ a moving target that generic nutrition advice simply doesn't track.
The inequality multiplier
Every risk above concentrates where food access is worst. Food insecurity during pregnancy is associated with higher rates of anemia, gestational diabetes and low-birth-weight deliveries โ the constraints we describe in our article on nutrition in low-income communities land hardest on pregnancies. Programs like WIC exist precisely because maternal nutrition is one of the highest-return investments in public health.
